Draft — effective date to be set by the practice
These Terms of Use explain the rules for using the website of 1234 Ortho-K Vision Care (“we,” “us,” or “our”). By using this website, you agree to these terms. If you do not agree, please do not use the website.
General information only
This website shares general information about our practice, orthokeratology (Ortho-K), and children’s eye care. It is not medical advice. It is not a substitute for an eye exam or for advice from Dr. Paul T. Woo, Optometrist, or another qualified eye care professional.
Every child’s eyes are different. Please do not start, stop, or change any treatment based only on what you read here. Talk with an eye doctor about your child’s specific needs.
No doctor–patient relationship
Reading this website, sending a booking request, or contacting us through the website does not create a doctor–patient relationship. That relationship begins only after you or your child is seen and accepted as a patient at our office.
In an emergency, call 911
This website is not for emergencies. If you or your child has a medical emergency, call 911 or go to the nearest emergency room right away.
If you have a sudden change in vision, eye pain, or an eye injury, do not use the booking form or wait for a reply. Call our office at (626) 282-5388 during office hours, or get emergency care.
Booking requests
Our website has a form you can use to request an appointment. The form asks for a parent or guardian’s name, a phone number, a preferred location, and a preferred time.
- Sending the form is a request only. It is not a confirmed appointment.
- Your appointment is set only when our office contacts you and confirms a date and time.
- We cannot promise that a requested location, date, or time will be available.
- Please do not include medical details in the form. Share health information with us by phone or in person instead.
The form is meant for parents, guardians, and other adults. Children should not submit information through this website. To learn how we handle the information you send, please read our Privacy Policy.
