Draft — effective date to be set by the practice
This policy explains how 1234 Ortho-K Vision Care ("we," "us," or "our") handles information from people who visit this website. It covers this website only.
If you are our patient, the health information we hold as your eye care provider is covered by our separate Notice of Privacy Practices, not by this policy. You can ask our office for a copy at any time.
Information we collect
Information you give us
You can browse this website without telling us who you are. If you use our booking request form, we collect:
- The parent's or guardian's name
- A phone number
- The office location you prefer
- The day or time you prefer
The form does not ask for medical details. Please do not include health information in the form. If you have questions about your child's eyes, call our office instead.
Information collected automatically
Like most websites, the services that run this site receive some technical information when your browser loads a page. This includes your IP address, browser type, device type, the page you asked for, and the date and time. These services use it to deliver pages, keep the site secure, and fix problems.
We use Google Analytics to count visits and see which pages are useful (see "When we share information" below). We do not use advertising cookies or tracking pixels. We do not create user accounts, and we do not take payments on this website.
How we use your information
We use the information from the booking request form only to:
- Call you back about your request
- Schedule a consultation or appointment
- Answer the questions you ask us
We do not use it for advertising, and we do not add you to a mailing list.
When we share information
We do not sell or rent your personal information. We do not share it with anyone for their own marketing.
We use a small number of service providers to run this website. They handle information only to provide their service to us:
- GitHub (GitHub Pages) hosts the website's pages.
- Cloudflare runs the program (a Cloudflare Worker) that receives your booking request and passes it to our office.
- Google Fonts supplies the typefaces the site uses. When a page loads, your browser asks Google's servers for the fonts, so Google receives your IP address and basic browser information. Google's own privacy policy covers that information.
- Google Analytics counts visits and shows us which pages people use. It sets cookies in your browser and receives information such as the pages you view, how long you stay, your approximate location (city level), and your device and browser type. We have turned off Google's advertising features, and we do not send Google your name, your phone number, or anything you type into the booking request form. Google's own privacy policy covers that information.
We may also share information if the law requires it, such as to answer a court order, or to protect the safety of our patients, staff, or the public.
Other websites and services
Apart from the services listed above, we do not allow other companies to collect information on this website about what you do over time or across other websites. This website may link to other websites, such as a map or a social media page. Those sites have their own privacy policies, and we are not responsible for them.
How long we keep information
We keep booking requests only as long as we need them to reach you and schedule your visit, and then we delete them. If your child becomes our patient, the information becomes part of the patient record and is kept as the law requires under our Notice of Privacy Practices.
Our service providers keep their technical logs for their own limited periods, as described in their privacy policies.
How we protect information
This website uses an encrypted connection (HTTPS), so information you send through the form is protected while it travels. We limit access to booking requests to office staff who need them to do their jobs.
No website or internet connection is completely secure, so we cannot promise that information will never be lost or misused. If you have a sensitive question, please call us.
Children's privacy
Our practice cares for children, but this website is meant for parents and guardians. It is not directed to children, and we do not knowingly collect personal information from children under 13 through this website.
The booking request form should be filled out by a parent or guardian. If we learn that a child under 13 has sent us personal information through the website, we will delete it. If you believe this has happened, please contact us.
Do Not Track signals
Some web browsers let you send a "Do Not Track" signal to the websites you visit. There is no common standard for how websites should respond to this signal, so this website does not change how it works when it receives one.
We use Google Analytics only to understand how this website is used. We do not use advertising tracking, and we do not follow your activity across other websites, whether or not your browser sends this signal.
Your choices
- You can use this website without sending us any personal information.
- You can call us instead of using the booking request form.
- You can ask us to see, correct, or delete the information you sent through the form.
- You can set your browser to block web fonts from outside sources. The site will still work, but the text may look different.
- You can block cookies in your browser settings, or install Google's opt-out add-on (tools.google.com/dlpage/gaoptout), to stop Google Analytics from counting your visits.
Changes to this policy
We may update this policy from time to time. When we do, we will post the new version on this page and change the effective date at the top. If we make an important change, we will also place a notice on our home page for a reasonable time.
